Re: account number [Account number]

Dear [Card issuer's name],

I am writing to dispute a billing error on my statement dated [Statement date].

The charge is for [Amount] on [Charge date], from [Merchant]. I did not make or authorize this charge.

Please correct this error, credit my account for the disputed amount and any related finance or other charges, and send me a corrected statement.

I understand that while you investigate, I do not have to pay the disputed amount or related charges, and that you may not report that amount as delinquent (12 CFR 1026.13).

Sincerely,

[Your name]

Fill in the highlighted blanks before you send it.

Before you send it

  • It must reach the issuer within 60 days after the first statement that showed the error.
  • Send it to the billing-error address on your statement, by certified mail with a return receipt. Keep a copy.
  • Keep paying the rest of your bill on time. Send copies of receipts, never originals.

Nothing you type here is saved or sent anywhere.
